Verification of Fee Waiver Eligibility

The UCSD admissions staff will verify the graduate school preparation program information you provided in your application. If this verification cannot be completed, you would then be contacted by e-mail for the following information. Only fill out this form if you have been asked to do so by graduate admissions. Note: all fields are required.

If you have already submitted your application using the fee waiver, you may change your decision. Mail the $60 application with the Fee Waiver Verification form (Form 1). (Word) (PDF) Your application will immediately be activated for processing.

If we have not received your reply e-mail within 5 days, a letter will be mailed to you. No action will be taken on your application until verification for a waiver is received and approved or the $60 fee is received.
